Before your first visit

Know what to expect.

Clear expectations make it easier to decide whether our model fits you—and help you get more value from your first appointment.

Your first steps

  1. Contact the clinic. Call or text 480-331-5044, or use the short request form on this site. Briefly tell us your main concern, goals and preferred clinician, if any.
  2. Choose the right visit. We screen each request rather than offering unscreened online booking. Our team will help determine the appropriate visit type and explain current fees and scheduling.
  3. Complete forms and send records. Provide the history and records requested before your appointment whenever possible.
  4. Meet with your clinician. Your visit may include detailed history, focused examination, review of records and discussion of initial priorities.
  5. Build the plan. Depending on complexity, recommendations may begin at the first visit or after records, labs or imaging are reviewed.

What to bring

  • A current list of prescriptions, over-the-counter medications and supplements with doses
  • Relevant laboratory and imaging reports
  • Important specialist notes or prior treatment records
  • A short timeline of your concern and treatments already tried
  • Your top three goals or questions

Insurance, HSA/FSA & payment options

Clinic visits and procedures are not billed to insurance, and we do not provide superbills. Commercial insurance often covers eligible outside laboratory testing, imaging and prescriptions according to your plan's rules. HSA/FSA plans often reimburse eligible care; please confirm with your administrator.

Payment plans may be available individually and through Advance Care. We also offer discounts for veterans and students on qualifying services—ask our team for current details.

Costs and decisions

Ask for current visit and procedure pricing when scheduling. When a plan includes optional testing or treatment, we want you to understand the purpose, alternatives and anticipated cost before proceeding.

For injection consultations

Bring imaging reports and, when available, the images themselves. Include a timeline of injury or onset, prior physical therapy, injections, medications, surgeries and what activities you want to regain. Do not stop prescriptions or supplements unless your clinician gives individualized instructions.
